Residential Program
OUTREACH Community Living Services, Inc. provides residential treatment (Portage Place), residential care (Oak Hill Rd.) and community housing services to persons in the Wayne/Holmes county area with a mental health diagnosis, mental retardation or to persons with both disorders who are in need of 24 hour supervision and monitoring. For persons who are dually diagnosed, OUTREACH Community Living Services, Inc. works closely in conjunction with The Counseling Center of Wayne and Holmes Counties and the Wayne County Board of MRDD to ensure that appropriate services are rendered. Referrals for services are received from various community agencies, family and other interested parties. Admission criteria include the following:
- Age 18 or older;
- Resident of Wayne or Holmes County;
- Own guardian, or agreement with guardian to participate in services;
- Documentation of disability;
- Behavior or actions which do not pose a threat to others or self;
- Willingness to provide information which will allow appropriate service delivery;
- Willingness to actively participate in development of an Individual Services Plan (ISP).
Services are delivered in the residential home and are designed to meet the needs and preferences of the person served. OUTREACH Community Living Services, Inc. ensures that 24-hour supervision is available to the person served, except when the Individual Service Plan allows for less-than-24-hour care.
The Residential Program’s residential treatment and community housing services promote the highest level of independence appropriate to each person. Upon admission to the group home and completion of a skills assessment, an Individual Service Plan (ISP) is developed with the active participation of the person. Linkage to other OUTREACH Community Living Services, Inc. programs as well as referral to other community resources are made as needed. Skill development and skill monitoring is offered in the areas identified and/or requested by the person. At the person’s request OUTREACH Community Living Services, Inc. may be appointed the representative payee for the person’s income. The person is expected to pay rent if living in one of the homes owned by OUTREACH Community Living Services, Inc., and may need to pay for additional services in certain residential locations when the individual income exceeds the sliding fee scale limits. Staff members also provide advocacy and assistance to the person in medical, legal, financial, transportation, benefit maintenance and other community living areas, as identified in the ISP. ISPs are written and reviewed in accordance with the state of Ohio Medicaid standards. The person is an active participant in the achievement of the goals and objectives identified on the ISP.
Increased independent functioning will be monitored through face to face, regularly scheduled meetings of the OUTREACH Community Living Services, Inc. services provider and the person. Community opportunities and activities are offered as appropriate and available. |
